Characterizing plausible causal pathways between COVID-19 reinfections and symptom ...
COVID-19 reinfections are prevalent, necessitating an urgent understanding of their cumulative, long-term impact in highly vulnerable populations. Evidence suggests that reinfections are associated with worse health outcomes; causal artificial intelligence (AI) modeling may provide additional insights. Using longitudinal symptom, biometric, and exertion data from 1,167 people with Long COVID and/or myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) in the Visible app, we investigated how cumulative COVID-19 infections may influence symptom trajectories. We evaluated five causal discovery algorithms with Non-combinatorial Optimization via Trace Exponential and Augmented lagRangian for Structure learning (NOTEARS) nonlinear emerging as optimal, showing strong predictive performance and structural validity. To evaluate pathways, we estimated interventional effects using the fitted structural causal model. Within our structural causal framework, our analysis suggests a 1.5% increase in post-infection symptom severity density and 4.9% increase in acute resting heart rate (RHR) across infections one to six. We estimate that pre-infection functional capacity (FUNCAP) may reduce post-infection symptom severity and symptom frequency by 27% and 21%, respectively, while pre-infection crashes may increase symptom severity by 8%. Due to symptom tracking requirements, this cohort was subject to a healthy user survivor bias, potentially underestimating true effects. Large-scale whole-genome sequencing reveals the landscape and health implications of de novo mutations
We performed WGS of 8,328 live-born infants from 7,851 completed parent−offspring families after quality control (Fig. 2), including 7,374 singleton and 477 twin pregnancies, yielding 24,030 analyzed samples. Among these families, 5,619 conceived naturally (NC) and 2,232 conceived through ART (Extended Data Table 1). We identified 390,924 high-quality autosomal dnSNVs and 28,032 de novo indels (dnIndels), corresponding to an average mutation rate of 1.14 × 10−8 per nucleotide per generation, within the estimated ranges between 1.00 × 10−8 and 1.29 × 10−8 from previous WGS studies4,7,15,18. Of these, 42,811 (10.22%; 39,330 dnSNVs and 3,481 dnIndels) with variant allele frequencies (VAFs) below 0.35 were classified as EPZMs. For the remaining 376,145 DNMs, parent of origin was determined for 39.18% DNMs (147,372), yielding 112,037 (29.79%; 104,954 dnSNVs and 7,083 dnIndels) paternal and 35,335 (9.39%; 33,040 dnSNVs and 2,295 dnIndels) maternal DNMs. In subsequent analyses, DNM counts were standardized by callable region size, resulting in an average standardized DNM count of 61.61 per offspring.
