A new Bloomberg investigation, citing U.S. officials involved in an internal Pentagon review, points to a series of preventable failures. A core issue was an overreliance on an AI tool developed by Palantir, called the Maven Smart System. This AI platform was intended to accelerate intelligence analysis and decision-making for U.S. Central Command. Officials suggest that some users expected Maven to automatically identify outdated or contradictory intelligence for potential targets, a function it was not designed for. The elementary school, mistakenly cataloged as an IRGC facility due to old data, was presented by Maven as a recommended target. This compressed targeting processes from hours to minutes. Palantir asserts no responsibility for the data or intelligence deficiencies, stating there is no evidence their software was flawed. They maintain the government holds primary responsibility for input data quality. Following the incident, Palantir enhanced Maven to re-review intelligence and flag disqualifying factors or inconsistencies. The rapid pace of over 1,000 targets in the first day also limited confirmation time. Furthermore, civilian harm mitigation teams within the Department of Defense have significantly shrunk, with U.S. Central Command's team reduced from ten to just one person. No member of these teams reviewed the Minab site before the strike. A United Nations inquiry concluded there were reasonable grounds to believe the U.S. committed a war crime through an indiscriminate attack.
