Re: Muste et al.: Outcomes and complications of pars plana vitrectomy for nonclearing vitreous hemorrhage without tractional elements (Ophthalmology Retina. 2026;10:596–605)

In a large, 306-eye, retrospective study of patients undergoing pars plana vitrectomy for nonclearing vitreous hemorrhage with no tractional component, Muste et al1 found that preoperative anti-VEGF use improved visual outcome but did not lead to fewer recurrent vitreous hemorrhages or fewer repeat vitrectomies. This is a useful clinically based data set, and the authors honestly acknowledge some of the limitations of it.

Submacular Hemorrhage Secondary to Cytomegalovirus Retinitis after Chimeric Antigen Receptor T-Cell Therapy

A 61-year-old woman with multiple myeloma received idecabtagene vicleucel, a B-cell maturation antigen-directed chimeric antigen receptor T-cell therapy, and was receiving foscarnet for cytomegalovirus viremia. She had thrombocytopenia (1.0 × 104/μL) and superior visual field loss in the left eye. Fundus photographs showed retinitis and subretinal hemorrhage; aqueous polymerase chain reaction detected cytomegalovirus DNA (1.0 × 106 copies/mL) (A). Seven days later, the hemorrhage extended subfoveally (B), as confirmed on OCT (C).

Dark adaptation (DA) has emerged as a potential functional outcome for assessing age-related macular degeneration (AMD). While reticular pseudodrusen (RPD) have been linked to poorer DA, most studies to date have focused on qualitative RPD presence/absence or comprised small cross-sectional cohorts. This work aimed to assess how automated quantification of RPD area and volume relates to rod-mediated DA and its longitudinal decline in non-advanced AMD.
