It's #Jan6th. 4 years ago, we were up to our knees in the #COVID #ICU. My fellow had exclaimed, "Omg, the capital is being attacked!", I paused, then said "We need to intubate Mr X and Ms Y". I came home to a despondent husband who looked like he had lost all faith in humanity. I hate that day.

The patient's husband called me a few days later to thank me & tell me his kids (9 & 6yr old) now want to be either a doctor/nurse when they grow up. We are not often able to say that families of a patient who died in the #ICU walk away with good memories, so this felt really nice. #3wishesprogram

The dying patient's son was turning 9 in a few days & had expressed that he was hoping to have his mother present at his party. Unfortunately, that wasn't going to happen. Instead, with his parents' permission, we had a small early birthday party for him in her room. Final #memories are precious.

Mom called at 7pm to check on me regarding the fires. Me: Yes, I'm at the hospital Mom: What?? are you ok? It's wild that after all these years, this response still causes alarm considering that I spend much of my life at the hospital as an #ICU doc.

Yesterday, in the chaos of patients crashing & needing the #ICU, another patient was improving & her husband said to me, "Thank you for saving my wife's life" The significance of these words are just sinking in now. It's easy to focus on disasters, but I must also remember our successes. #medsky

Today, I saw in clinic a patient who was on life support for months in our ICU. Walking, talking, eating, LIVING. Wins in the #ICU are often so few and far in between, I couldn’t help but cry. I can’t tell you how much I needed this.

As #ICU docs, we often rotate off service in a week. But it's an intense week, such that close relationships with patients/families are often developed. Today, I learned that one of the patients I cared for a few weeks ago had died. I called the family to offer my condolences and (1/2)

During the last #Trump administration, I admitted a patient to the #ICU for monitoring after a head concussion. The nurse did her assessment & asked, "Who is the president?" Patient replied, "Putin" and nurse replied, "Yep, Alert and oriented x4". I can't believe we're here again.

Last week, I was on a crazy busy #ICU week, was miserably sleep-deprived, and would fall asleep as soon my head hit the pillow each night. This week, I toss & turn thinking about the end of science, medicine, and America. I couldn't fall asleep last night even after taking an Ambien. Sigh. #medsky
