Centralized Multidisciplinary Retinoblastoma Care Closes the Outcome Gap: Survival and Globe Salvage for 1000 Eyes from 601 Patients

Retinoblastoma (RB) outcomes in low- and middle-income countries remain inferior to those achieved in high-income countries, largely due to delayed diagnosis, limited access to specialized care, and fragmented treatment systems. The aim of this study is to evaluate the long-term impact of a two-decade strategy based on centralized multidisciplinary care, structured capacity building, and a telemedicine-supported international twinning partnership in reducing disparities in retinoblastoma (RB) outcomes in a resource-limited setting.

Mind the Gap: The Urban-Rural Divide in Diabetic Retinopathy Care in the United States

In 2026, diabetes mellitus affects over 40 million Americans, nearly 12% of the population, and diabetic retinopathy (DR) remains the leading cause of blindness among working-age adults. Although there have been significance advances in the treatment of DR involving anti-VEGF therapy, challenges with adherence limit its effectiveness in clinical practice.1 These concerns are magnified in non-urban areas where socioeconomic constraints play a larger role and access to care is limited by geography and specialist availability.

Nonarteritic Anterior Ischemic Optic Neuropathy and Glucagon-Like Peptide-1 Agonist Therapies

Robertson et al1 report on the cup-to-disc ratio (CDR) in the unaffected fellow eyes of 2169 patients with nonarteritic anterior ischemic optic neuropathy (NAION), including those with and without prior glucagon-like peptide-1 receptor agonist (GLP-1 RA) exposure, retrospectively drawn from 16 neuro-ophthalmology centers, compared with a control group of 465 individuals from a separate normative imaging data set. The report concludes that eyes with a CDR ≤0.20, as measured by fundus photography, have 46 times the odds of developing NAION compared with eyes with a CDR >0.40.

In this retrospective case-control study, Elaraby et al (p. 1185) investigated the potential impact of ocular inflammation on development of anti-adalimumab antibodies (AAAs) in patients with autoimmune diseases treated with adalimumab. Analysis of 704 enrolled patients found AAAs in 151 patients (21.5%). Ocular inflammation, female sex, and adalimumab interruption were independently associated with higher odds of positive AAA findings. Prior exposure to other tumor necrosis factor inhibitors did not reduce the likelihood of AAA formation, but prior treatment with biologics targeting non-TNF pathways was associated with a significantly lower risk of AAA development.
