Over 1,000 ships are sitting on the hook and this guy for some reason thought he was special. This captain unnecessarily put his crew and cargo at risk and now three are missing. Plus, he put at risk the other crews who had to conduct search and rescue. He needs to be held accountable.

A full and transparent investigation won't prevent this from happening again. The 9/11 Commission was that for 9/11. It made recommendations and we created laws as a result. Trump has ignored many of those laws. I'm not even sure if the CIA and the FBI are effectively communicating right now.

Killing civilians by accident in war is not automatically a war crime. But failing to properly verify a target, or causing clearly excessive civilian casualties, can be. "Reasonable steps" must be taken to differentiate between targets of military value and civilians.
